DESCRIPTION:The Rock County Health Department will be conducting an Immunization Clinic in Milton at the Milton United Methodist Church\, 241 Northside Dr.\, from 3- 5 p.m. on Tuesday\, March 13\, 2012.\n\n\n\nParents need to be present to sign forms before vaccines can be given. Please bring a record of when your child has received other vaccines. The following vaccines will be available at this clinic: DTaP\, Polio\, Tetanus\, MMR\, Infant Hib\, Hepatitis B\, Menactra\, HPV\, Varicella\, Rotavirus (RotaTeq) and Prevnar. The cost of each listed vaccine for a person birth through 18 years of age is $5.00. For a person 19 years of age and older\, the cost of the vaccines vary. \n\n\n\nVaccines may also be obtained at both Rock County Health Department Offices. The\n\n\n\nJanesville Office is located at 3328 Hwy 51 North\, Janesville and the Beloit Office at 61 Eclipse Center\, Beloit. Clinic hours at both offices are Monday\, Wednesday and Friday from 8:30 to 10:30 a.m. by appointment only. At the Janesville office Friday from 10:30 a.m. to 4 p.m.\, no appointments are necessary. At the Beloit office Wednesday from 10:30 a.m. to 4 p.m.\, no appointments are necessary.\n\n\n\nThe Rock County Health Department also offers free immunization consultations to parents who would like to know what immunizations their child may need.
